SHORT ARM THUMB SPICA

Indications: Scaphoid fracture

Bennett’s fracture

Other thumb fx

Tendinitis

Materials needed:

1” Stockinette for thumb

2” Stockinette for arm

2 or 3 rolls of 2” Webril

2 Rolls of 2” Fiberglass

Casting Gloves

Scissors

POSITIONING:

For scaphoid fractures... Thumb MUST BE in alignment with forearm


Thumb MUST BE in alignment with index finger (like holding a can of soda or cup of coffee)

Application: Place the thumb stockinette over the entire thumb. Place arm stockinette. Start webril as you would with a short arm cast. Start with 2 or 3 base rolls around the base of the wrist, bring webril up and thru the thenar space, around hand, thru the thenar space again, around hand then direct webril starting around base of thumb. Work your way up around the thumb, then come back down, around  bottom of hand then continue down forearm as you would a short arm cast.  Start your 2” fiberglass following the same pattern as the stockinette. Once you finish your first roll of fiberglass, flip the ends of your stockinette over including the tip of the thumb. Proceed with your second roll of fiberglass until you have finished that roll.  Gently mold the forearm as you would the short arm cast. Follow alignment recommendations of thumb as discussed above.
